The atlas (C1) and axis (C2) normally form a pivotal joint that allows free movement of the head along the longitudinal axis of the spine. Atlanto-axial instability usually occurs secondary to congenital or developmental abnormalities of the bones or ligaments of the atlanto-axial joint, traumatic injury of the joint, or a combination of both. Dorsal displacement of the cranial portion of the body of the axis into the vertebral canal, acutely or chronically, causes compression, edema, and inflammation of the spinal cord. Clinical signs of C1-C5 myelopathy and/or cervical pain occur and may be acute or chronic in onset, and progressive, non-progressive, or intermittent in nature. This presentation will discuss the diagnosis and treatment options of this condition.
