Neuromuscular diseases can produce some classical clinical abnormalities making them a fascinating group of disorders. Most typically the gait abnormalities seen are often dramatic and get worse before our eyes with minimal activity. However, the clinical signs are not specific for the underlying cause and can leave us frustrated. Confirmation of these disorders often requires consideration of electrophysiology and histopathology of muscle and nerve biopsies. This requirement can be a problem in many practice environments and so we will look at how we can approach these cases without the aid of these expensive and invasive diagnostics. We will use video footage of cases to highlight the important parts of their clinical examination and then discuss differential diagnoses, diagnostic plans and treatment options.
