A Step-by-Step Guide to Fixing Lateral Humeral Condylar Fractures

Fracture of the lateral portion of the humeral condyle (FxLHC) is the most common fracture of the canine humerus. Salter-Harris Type IV FxLHC has a peak incidence at 4 months of age and often occurs with minimal inciting trauma. In the mature dog, incomplete ossification of the humeral condyle (IOHC) can be a predisposing factor, particularly in spaniels.

The clinical and radiographic diagnosis of FxLHC will be reviewed, and complicating factors e.g. fracture chronicity, IOHC, bicondylar fracture, and fracture of the medial coronoid process, will be discussed. The utility of computed tomography in the detection of IOHC will be demonstrated.

Surgical repair of a simple FxLHC using a transcondylar lag screw and anti-rotational K-wire, via a caudo-lateral approach to the elbow, will be illustrated with a technical emphasis on improving the consistency of intra-articular fracture repair.
